When and how to report
What to report
You should report any suspected or actual violations of laws or regulations, our Code of Conduct, Octave policies or our Supplier Code of Conduct. This includes concerns involving Octave, Octave suppliers or any Octave business partners.
What to expect
All reports are handled confidentially and reviewed through a fair, objective process. We strictly prohibit retaliation against anyone who reports concerns in good faith. After submitting a report, you’ll receive a case ID so you can track its status and provide additional information if needed.
Each report is carefully assessed and may lead to an investigation, referral or closure depending on the findings. When appropriate, corrective action will be taken and feedback may be provided.
When to use other channels
For routine operational or customer issues, or personal employment disputes, use HR, line management or the relevant operational function.
